Session 1 – Timing of implant placement: can we accelerate treatment with immediate placement?

Abstract

This lecture will explore the considerations and patient-specific strategies in the election of delayed versus immediate implant placement. The decision-making process between these two approaches involves various factors, including bone quality and quantity, soft tissue conditions, patient health status, and esthetic demands. Understanding the advantages and limitations of each approach is crucial for clinicians to provide optimal treatment outcomes. Through a comprehensive analysis of existing literature and clinical experiences, this lecture aims to provide insights into navigating the complexities of delayed and immediate implant placement, offering valuable guidance for dental practitioners.

Compromised fresh extraction sites in anterior locations present a real challenge to many clinicians, the treatment decision is often in the favor of delaying implant placement (the early placement protocol) that claimed that immediate implant placement in compromised sites did not offer any clinical reliability and posed higher risk to the overall treatment outcome. However delaying implant placement has shown several drawbacks that include; the drop of both vertical and horizontal socket profiles, the higher tendency to post restorative recession as a result of the mucoperiosteal flap reflection, longer treatment time, multiple surgical innervations, the hardship of fabricating several provisional restorations during the treatment time , and the poor esthetic results specially when restoring  continuous adjacent sockets or in thin phenotypes patients  .  This presentation describes a novel clinical protocol to treat wide varieties of fresh extraction sites (thin, or deficient labial plate of bone, severely compromised, actively infected) using a single minimally invasive techniques that allows immediacy, shorten the treatment time to only 8 weeks from the start to the final prosthetic insertion. The protocol is designed to rehabilitate the defective socket environment while placing immediate implant at the same surgical setting thus providing immediacy. The VST protocol is an evidence based concept drawn from an extensive research work conducted in the past 5 years , the VST offered a spermine esthetic outcome, long term stability of the marginal tissues,  preserved the original socket topography during the steps of the treatment until the treatment completion, no mid facial post restorative recession, and offered a long term predictable clinical outcome in sockets presented with active signs of infection .

Many patients with adjacent missing teeth in the “esthetic zone” present to us with less-than-ideal alveolar ridge form due to severe hard and soft tissue loss. While many techniques offer excellent results for reconstruction of these defects, few can be said to guarantee success.  A bone or soft tissue augmentation procedure to correct a ridge defect in the esthetic zone that is not fully successful in correcting the defect often ensures the use of prosthetic pink. This leads to the treatment dilemma: “if we have to use a little pink, why not use a lot of pink” and spare the patient the painful augmentations in the first place? The aim of this lecture is to critically evaluate the current evidence to determine the predictability of various bone augmentation techniques for vertical ridge defects for implant placement and when to consider more conservative alternative treatment options. A series of cases have been gathered to illustrate risk assessment and predictable management of various critical size defects ranging from moderate to severe defects.

How can we develop an efficient and consistant treatment concept – important steps with and without patient even nowadays. Is it expedient to remove all teeth and do a all on 4 or all on 6 planning. Can we promise a long term stability to the patient. Why not preserving teeth and implement them in a clever treatment concept? With their own teeth patients are able to feel, have a better feedback while chewing, biting or even speaking. The digital workflow helps us today to visualize our treatment plan and for implant surgeries where is the best position for an esthetic and long term stability and outcome.

How much patient is needed for a predictable final result – which information are necessary to reduce practice time and increase efficiency with a digital workflow?

Digital technologies tremendously increased the predictability of surgical and prosthetic techniques. Different techniques and devices are recommended for the accurate digital impressions. However, digital impressions represent only one aspect among of others that influence the fit of the final prostheses. Moreover, the fit of the prosthesis is mainly regarded as the passive-fit. Other aspects of the fit that have tremendous effect on clinical effectiveness of digital workflow are often not considered. This lecture will cover the factors that establish favorable conditions to achieve different aspects of the fit and to increase the effectiveness of digital workflow.

  • The concept of immediate loading after immediate placement has been modernised to include simultaneous soft tissue grafting (surgical veneer) especially in the aesthetic zone.
